Your Visit to Our Office

  • If this is your first visit to our practice, welcome. The West Clinic is devoted to providing the most comprehensive medical care in the most comfortable environment possible.
  • If you are a new patient, you will be asked to complete a set of forms that will provide us with the information we need to give you our best care. This packet will include information regarding your medical history, a medical release form, any medications you are taking, and a patient information sheet. It is important that you bring your insurance cards with you.
  • Don’t forget to let us know whenever you change your address, telephone number, employment, marital status or insurance.
  • Please make your next appointment every time you visit the clinic. This includes lab, medication and radiology appointments.
  • Please sign in at the front desk every time you visit the clinic. Even if your visit is for lab, chemo or radiology, signing in at the Front Desk will let us know that you have arrived.
  • If your physician is unavailable, we have a Nurse Practitioner on site to assess and evaluate your condition.
  • If you are coming to The West Clinic from out of town and need additional information or special assistance, please call our Humphreys Site Director at 683-0055, ext. 1102.

Calling our Office

  • At times it will be necessary for you to reach your doctor or nurse. During office hours (M-F, 8:30 – 5:30) you should call the office where you see your physician. An operator will take your message.
  • In order to allow the doctors to devote their full attention to scheduled patients, a nurse will initially handle your call. Our nurses have special training to enable them to help you with questions or problems that may arise. The nurse will also consult with your physician.
  • Please be patient with us as we receive many calls during the day. Your call will be returned as soon as possible.
  • If you are experiencing an emergency, such as chest pain or shortness of breath, call 911.
  • After hours, your call will be answered by our professional answering service and the doctor/nurse on call will return your call as soon as possible.

Prescription Refills

  • If possible, get your prescriptions filled while you are in the clinic. If you are unable to do so, please call between 9:00am and 3:00pm for your refill.
  • All prescriptions will be called in to your pharmacy by 6:00pm on the day of your call.
  • Remember that most narcotic prescriptions require a written prescription.
  • When calling in for a refill, the operator will take a message and a nurse will return your call if necessary. Please have the name of the prescription, the dosage, and your pharmacy number available.

Test Results

  • All patients are anxious to know the results of any tests that are performed. Please know that you will receive your results as quickly as possible.
  • Since many of your tests will be performed at The West Clinic, we will be able to give you the information you need promptly.

Medical Records / Release of Information

  • We regard the doctor – patient relationship as sacred. The contents of your medical records are confidential and will only be released to outside sources upon an express written authorization.

Smoking Policy

  • The West Clinic is committed to the health and well-being of its patients, guests, and staff members, and as such, smoking is prohibited in all offices of The West Clinic.
